What should my eviction notice say if i'm applying for temporary assistance?

I am 24 and currently living with my mother and am on the lease, i am currently waiting on a decision from disability and am not sure when i will be able to work full time again. i would like to apply for temporary assistance since my mother is struggling to support me. i know a eviction letter would be good to have, just wondering what it should say so i don't look bad when i try to rent some where else
